You send a data packet with your internet, a remote computer (the server) receives it, analyzes it, and sends it everyone else. This data can be your location, ore you've dug up, or whatever else other people should see you doing. If there is lag it is probably because of the server speed or your internet speed.
That's why PC gamers always cry when there isn't dedicated server support for online games. Dedicated servers run incredibly quickly and way faster than an average person could setup. This way the lag is always the users fault, not the servers.
In this case it is probably the server and certainly not Minecraft. You can make online play more efficient with better coding, but code can only do so much.
